If the date on your warranty card is more than 2 years ago for the All Purpose Insoles or more than 3 years ago for the Sports Insoles, your warranty has expired. Please purchase another pair. If you do not know the date of purchase or you cannot find your warranty card, you must include a copy of the original receipt, a purchase confirmation or warranty email, or a copy of a credit card statement showing FOOTRELIEF as the name with date on the line item.

Terms and Conditions Required to Receive a Warranty Replacement

I understand and agree that I must fill out this RMA Warranty Replacement Form and include a printed copy of the RMA Form (found in your email after submitting), along with both left and right insoles, and include with my shipment at least one of the following proofs of Purchase: a) The Original Dated Warranty Card (this will be returned to you), b) Copy of Dated Sales Receipt, c) Copy of Dated Email Receipt, d) Copy of Dated Warranty Registration Email, e) Copy of Dated Credit Card Line Item Showing FOOTRELIEFINSOLES with a Date of Purchase clearly shown on the Line Item. I also understand and agree that the 100% warranty coverage only applies to sudden loss of liquid from seal failure due to normal wear. To receive a replacement for defective insoles less than 90 days from the date of purchase I must send a check or money order payable to Foot Relief Insoles LLC in the amount of $6.95 per pair for the All Purpose Insoles or $8.95 per pair for the Sports Insoles to cover return shipping and handling. To receive a replacement for defective insoles more than 90 days from the date of purchase I must send a check or money order payable to Foot Relief Insoles LLC in the amount of $9.95 per pair for the All Purpose Insoles or $14.95 per pair for the Sports Insoles to cover return shipping and handling. I understand that the insole warranty is NOT 100% covered by the following: Cuts, Rips, Tears, Punctures, Chew Marks, Liner Wear, Improper Trimming, Melting, Burns, or any other non-manufacturer defect. I understand that for any non-manufacturer damage or wear I may still receive a replacement under warranty by including proof of Date of Purchase and a check or money order in the amount of $14.95 for the All Purpose Insoles, or $19.95 for the Sports Insoles payable to Foot Relief Insoles LLC. I understand that if I cannot locate or provide evidence of a proof of purchase date that I may receive a one time courtesy replacement for defective, worn, or damaged insoles by sending both left and right insoles, a copy of the RMA Warranty Replacement Form, and a check or money order payable to Foot Relief Insoles LLC in the amount of $24.95 for the All Purpose Insoles or $29.95 for the Sports Insoles. This option is a one time warranty replacement only and the insoles you receive will have no additional warranty coverage since we cannot accurately determine when you purchased them. We are relying on your honesty in this situation.
