To be eligible for a
refund, credit reversal, or to stop a scheduled
payment, you must fill out the RMA form and follow
the requirements below. Once we receive your insoles by mail, you will receive a reversal of
all charges to your credit card or a scheduled
payment will be stopped, whichever applies. This RMA form is for
Refund Requests only.

Replacement form.
1. You must
fill out the online RMA Form (below) on or before 30
days from the date of purchase to be eligible for a
refund. The date of purchase is hand written
on your warranty card (Insole Package).
Customers who purchased their insoles at a retail
outlet who do not have a hand written date on the warranty
card must include a copy of a sales receipt or
a copy of the credit card statement showing FOOTRELIEF
in the transaction description.
2. You must
postmark your return on or before 30 days from the
date of purchase. Insoles postmarked after 30
days from the date of purchase will be returned to
the sender and are not eligible for a refund.

You must
include a printed copy of the RMA form results with your
return. A copy of the RMA form results will be sent
to your email once you complete the
form below.
4.
You must
ship both insoles, the dated warranty card, and a copy of the RMA form results on or
before 30 days from the date of purchase to be
eligible for a refund.
5. Please
wash insoles and air dry prior to shipping.

copy of the RMA form results by email. You
must print the RMA form results from your email
inbox and send a copy with your
refund request.
Please check your
email spam and junk box before contacting us if you do not receive
a copy of the completed RMA form results.
