To be eligible for a
refund you must follow all the requirements below to receive a reversal of
charges to your credit card or to receive a refund check.
By filling out this
RMA Refund Form you understand and agree that you
must send both insoles, the dated warranty card, and
include a printed copy of this RMA form with your
return (check your email once you hit submit).
You also understand and agree that you must postmark
your return on or before the 30th day from the date
of purchase to receive a refund or to stop an
automatic payment. Insoles postmarked after 30
days from the date of purchase will be returned to
the sender and are not eligible for a refund.
IMPORTANT: Customers who purchased their insoles
from an independent reseller or from an independent retail
outlet must send or return their insoles to that
location or business for a refund. Since we
did not process your order, we have no access to the
transaction and have no way of refunding you.

information if you did not purchase your products
directly from Foot Relief Insoles LLC.

lock package or zip lock bag.
